As a dedicated cinephile, I must say that the events leading up to the catastrophic car accident in this film unfold before anything else – even preceding the initial collision. However, the harrowing details of what transpired aren’t fully unveiled until Skye revisits that fateful moment repeatedly. The second car crash scene serves as a chilling portrayal of the incident that claimed Paul Hudson’s life and left Skye with unimaginable pain.
In the aftermath of the collision, Skye endures harrowing ordeals. It’s evident that Paul has perished, his upper body disfigured, with a portion of his head missing and his eyes wide open in a tranquil gaze. Conversely, Skye, though injured, is still alive, frantically calling out for assistance.
In this scene, it is most evident that the visual element of their tense situation unfolds during Skye and Paul’s heated argument prior to the car crash. Despite being obviously inebriated, the director cleverly manipulates the camera angle, stirring our apprehension since we understand what is going to transpire, but not exactly when. 4 The Opening Car Crash Sequence
In the initial scenes of the movie, we find Joel, the police detective from the earlier ‘Smile’ installment who miraculously survived, attempting to transfer the curse onto unsavory individuals who might justifiably bear it. However, his scheme doesn’t unfold as he intended.
Guns are discharged, and Joel’s scheme goes awry. A crucial character, a young man, appears on the scene, causing Joel to flee as additional crooks show up and unload their guns at him. Fortunately, he escapes the house, but tragically, a vehicle hits him in the process. At this point, we understand that the director hasn’t switched shots, and it’s all one continuous take.
As the camera pans back, we see the carnage. We hope Joel is alive, but the more blood we see, the more we lose hope. It’s not until the camera is fully positioned above the road that we see Joel’s mangled remains. It’s an early death that’s hard to recover from.
3 The Death of Skye’s Mother
It appears that Skye was discovered in her apartment, leading many to assume she might be abusing drugs. However, there are some doubts as to whether what we witnessed – her being encompassed by a dozen cheerful strangers who seemed intent on dismembering her – was actually real or just a vivid dream brought on by high fever.
In this scenario, Skye’s mom, Elizabeth, firmly persuades Skye to embark on her tour. Skye feels she’s experiencing some supernatural occurrence, yet everyone around her remains skeptical. While the situation may seem a tad foreseeable, it was still plausible for the unknown force not to reveal itself through anyone else. When the event occurs, one of the movie’s most gruesome scenes unfolds.
Elizabeth shatters a mirror, picks up a sharp fragment and starts brutally attacking herself, focusing on her neck and face. It seems like an out-of-control beast within her is intent on inflicting harm on its primary target. Meanwhile, Skye stands frozen in horror as she watches the scene unfold. However, she soon becomes aware that she’s holding the shard herself – it was her who ended her mother’s life, now lying motionless in a sea of blood with her upper body horribly disfigured.
2 Skye’s Death
In the midst of a performance, the singer unexpectedly finds herself awake, as if emerging from a dream, after previously being trapped inside a laboratory-like freezer room battling her sinister counterpart. Her mother enthusiastically applauds from an exclusive seating area, while Skye stands in bewilderment, unsure of how to respond, since the events leading up to this moment were merely hallucinations.
It seems that the peculiar figure with the wide grin is visible exclusively to Skye, who interprets this as a sign of her impending time. This figure apparently invades Skye by dismantling her face and passing through her mouth, which is the singer’s. The crowd merely hears her struggle and what sounds like choking as she appears to die.
However, following a momentary lapse, Skye regains her composure. She confidently steps forward amidst thousands, flashing a chilling grin that leaves no room for doubt about what follows. The audience’s discomfort escalates with each sound of Skye performing some action echoing throughout the venue. In the denouement, we discover that Skye had fatally stabbed herself through the eye, leaving her corpse with an unsettling smile etched on its face – a sight that sends shivers down the spine in this movie.
1 Lewis Fregoli’s Death
The movie’s most striking scene is undeniably the gruesome one featuring Lewis Fregoli’s demise and the unfortunate eyewitnessing of this event. This scene, which features weight plates, might give you pause before venturing to the gym tomorrow.

With an overwhelming force, Lewis starts pounding his face with the plate. Each strike is increasingly vicious, and the brutality of each blow is evident in what’s left of Lewis’ once-recognizable facial features after each impact. All that remains of him before Skye flees in terror is a gruesome mess of flesh and muscle – the last image Skye sees before taking flight.
